Puppy Application

Thank you for considering a puppy from Champaign Shih Tzu. We take great pride in raising happy, healthy puppies and are committed to helping each one find a loving forever home. To ensure the best fit for both our puppies and their future families, we ask a few questions about you, your household, and the environment your new companion will be joining.

When you’ve found the puppy you’d like to welcome into your family, a $200 deposit is required to reserve that puppy. Once the deposit has been received, the puppy will be marked as reserved and removed from availability. The state of Illinois requires taxes to be paid on all puppy sales.

Step 1: Complete the Puppy Application

Please fill out the application below with accurate information so we can learn more about you and your family.

Step 2: Reserve Your Puppy

After submitting your application, navigate to the puppy you have selected, add him or her to your cart, and submit the $200 reservation deposit. Please note that submitting an application alone does not hold a puppy. Puppies are reserved on a first-come, first-served basis once a deposit has been received. If another family places a deposit before you, you may need to choose a different puppy.
